As a nanny are you prepared for emergency situations? Do you have the following handy?
Emergency consent form / Authorization to treat a minor?
Copies of insurance cards – auto, health, prescription.
Contact numbers
- Parents – work, home, cell
- Neighbors
- Relatives
- Teachers
- Pediatrician
- Dentist
- School
Have multiple copies made and keep in car, purse, diaper bag / backpack, stroller, any item used on a daily basis with the children. Also, provide pediatrician and dentist with a copy of authorization to treat a minor form, so they have in their files.
I keep the forms in ziploc baggies, just to prevent water damage (or spilled sippy cups, etc). Also, check the laws in your state…as they vary. In Michigan, the consent form I carry with me is only valid for 6 mos.
